Note: To register, your child must currently be age 5 to Grade 5 (or if registering in the summer, going into Kindergarten to Grade 5).

WAIVER AND RELEASE FORM:

PARENTAL/GUARDIAN CONSENT: (Please note that there is always a possibility that injury or illness may occur while a child is attending Faithbuilders. Our team of volunteer staff will do our very best to care for a child in such a situation.)

WAIVER AND RELEASE: I, the parent/guardian responsible for this child, release Hillsdale Baptist Church of any liability related to the child’s participation in any or all of the activities offered at Faithbuilders from September 2023 through April 2024, held at 101 Munroe Place, Regina, Saskatchewan. I also understand that Hillsdale Baptist Church retains the right to use, for church information and Faithbuilders program purposes, photographs and video footage of children taken during Faithbuilders. These resources will not be used online or in public media.
